Simple registration

To make your first visit smooth, please plan to arrive a few minutes early so we can complete your registration paperwork together.

Bring your ID

Please bring a photo ID and your insurance card, if you have dental coverage.

Health history

Have a list of any medications and relevant medical history handy so we can care for you safely.

Arrive early

Arriving 10 minutes early gives us time to complete forms and start your visit on time.

Complimentary parking

Park free in the attached garage below the LUXE building. Designated spots are reserved for Ansley Midtown Dental patients, and our team will provide the guest parking passcode at check-in.
